The part after you hit your goal

Losing the weight and keeping it off are two different projects. The second one is the reason Axiom refers patients on at the end.

Obesity is a serious physical, mental and societal problem, and losing weight can be a very difficult process. Keeping it off is its own difficulty, and it is the one clinics talk about least.

Why the handoff exists

When you have achieved your weight loss or wellness goals, Axiom gives you referrals to its network of coaches, trainers and nutritionists. That is a deliberate part of the program rather than an afterthought bolted on when the last appointment runs out of things to say, and it is written into how the practice describes the program from the first consultation onwards.

The practice puts it plainly: through collaboration with our partners, we can help you achieve your results faster, and help you maintain your goals for the long term, in a safe and affordable way.

What to build while you are still on treatment

The habits that hold afterwards are the ones you started during, not the ones you plan for later. Movement you would do anyway, meals you actually like, and a routine that survives a bad week are worth more than a perfect plan you abandon.

Why maintenance is its own skill

Losing weight runs on a plan somebody else is monitoring, with a clear target and regular check-ins. Maintenance runs on habits you own, at a body size that needs less food than the one you started at, with no obvious finish line to aim at. Those are genuinely different projects, and the fact that the first one went well is not by itself a prediction about the second.

What to build while you still have support

The routines that hold afterwards are the ones you started while treatment was still doing part of the work: meals you actually like rather than meals you tolerate, movement you would do anyway, and a week that survives a bad Tuesday without unravelling. Protecting lean mass with resistance training matters here too, because muscle affects how much you can eat in maintenance without regaining. Ask about the referral network before the last appointment rather than at it.

Why the referral is not a goodbye

Being handed on to a coach or a nutritionist is not the clinic closing your file. It is the recognition that the skills maintenance needs are not the same skills a prescribing physician provides, and that the person best placed to help with the next year is somebody whose whole job is that year. Ask what the handoff looks like early rather than discovering it at the last appointment.
